NOTICE: THE CONFERENCE IS FULL AND REGISTRATION IS CLOSED AS OF DECEMBER 9, 2019. UNLESS YOU HAVE SPECIFIC PERMISSION FROM THE ORGANIZERS, DO NOT REGISTER ON THIS PAGE. IF YOU DO SO, WE WILL REFUND YOUR FEE MINUS A $50 ADMINISTRATIVE CHARGE.

Attendance fee, including CME/CE credits and lunch and snacks on both days:

Physician/pharmacist/NP&PA/administrator/director/academic: $500

RNs, SWs, chaplains, hospice staff: $400

Organizational Discount: If you have 10 or more RNs, SWs, or other direct-line clinical staff from your organization attending, a discount rate of $300 will apply. Contact [email protected] to access the discount.

All others, please contact [email protected]. Do not simply select the lower rate without checking with us first.

CANCELLATION POLICY
As of January 3, cancellations are subject to a $100 administrative fee. We cannot accept cancellations after February 10, 2020. Send cancellation requests to [email protected]
